PPE is small, often the same colour as the background, and frequently seen from above. The failure that costs you is a false pass, and false passes are the ones nobody reports.

The question is per-person, not per-frame. Keypoints anchor gear to a body so a helmet on a rail does not count as a helmet on a head, and so a harness can be judged against a torso rather than against the scene.
This is the decision that is expensive to reverse. The geometry has to match what the answer contains, and finding out it does not means labelling the set a second time.
